How to see your Doctor
Our appointment system aims to enable you to obtain an appointment on the day of your request whether or not your problem is urgent, and as far as possible with a doctor of your choice.
In order for this sytem to work successfully, we need your co-operation. Please help us by not requesting appointments several weeks in advance. A few pre-bookable appointments will be available, but most will only be made available the
same day. We would recommend therefore that you telephone or call at reception on the day you wish to be seen.
Consultations with the Doctor are usually between 08.15 am to 12.00 midday in the mornings and 3.30 pm to
6.00 pm in the evenings on weekdays. There may be some early afternoon appointments available. A Doctor is in attendance at the
surgery personally between these times.
We try to run to time as far as possible. Inevitably an appointment can run overtime, so please bear with us if you have to wait. In order to try and stay on time the Doctor may only be able to deal with one problem per appointment.
If you no longer need an appointment or cannot attend, please let us know so that another patient may use it.
We ask you to be as prompt as you can for your appointment and may ask you to re-book an appointment if you are more than ten minutes late.
We offer a confidential service to people of all ages. We would normally expect a parent/guardian to attend with younger children.

 | Home Visits
The doctors will visit you at home if you are seriously ill or confined to the house. Only in very exceptional circumstances should a house visit for a child be necessary.
We are sorry, but it is not possible to promise that one particular doctor will visit, but requests will be noted.
When you request a visit the Receptionist will ask for full details of the patient's name, address, telephone number and the reason for the visit. This information enables the doctors to prioritise their calls.
The doctor's time is valuable. Please bear in mind that a house visit takes four to six times as long as a consultation at the surgery.
Except in emergencies please telephone the surgery before 11.00 am for a visit. After 11.00 am please request a visit only in emergencies.
There is a doctor available to deal with emergency house visits. If you are telephoning the surgery for one of these, then you will be put through to the
duty doctor who will assess if a home visit is required.

 | Emergency Out of Hours
It is our aim to provide you with a good service and we hope to meet your expectations as a patient.
Consultation is by appointment only. Although it is best to see the same doctor for the
continuity of treatment, your doctor may not always be available on the same day and
time in which case you are free to see any of the other doctors. Please make a
separate appointment for each person.
Appointments may be made at the reception or by telephoning the surgery.
The Practice has a doctor available for emergencies at all times. Telephone: (01782) 512822.
A pre-recorded message will then give you a telephone number to ring (01782) 719100 where a doctor can be contacted. Please only use this service in cases of genuine medical need.
The Practice takes part in North Staffs Urgent Care and cover is from 06.30 pm
until 08.00 am on week nights and between 7.00 am on Saturdays to 7.00 am on Monday mornings and Bank Holidays. This is a comprehensive service performed by local General Practitioners.

 | Repeat Prescriptions
If you are on regular medication, the doctor may authorise a repeat prescription. This is so that you can obtain your regular prescription in between your regular check-up with your
doctor.
Repeat prescriptions can be ordered online from the online
services page, by post sending a stamped addressed envelope or at reception.
Setting up online prescription ordering:
We are pleased to announce the introduction of our repeat prescription
online requesting facility which does exactly what it suggests. Please
initially request a Registration Letter, which can only be
obtained in person at the practice. You cannot create an account without
one and (unless you are well known to the practice staff) you need
photo identification, ie a passport or new style driving licence,
in order for it to be issued.
Once you are registered
We are keen to improve access for our patients on regular repeat
"scripts" but also wary of the potential dangers of prescribing
non-regular scripts this way. Therefore we would remind patients that ONLY
regular repeat prescriptions can be ordered in this way and,
unfortunately, any attached messages to the request are unlikely to be acted
on (especially ones like "could I also have some more of those pink
tablets with black spots on?"!!). You can, however, let us know of any
of the three Biddulph or Biddulph Moor pharmacies that you'd like to collect
your script on your behalf.
You can log back onto EmisAccess via the same button to view the progress of
your script request at any time. Please remember scripts can take two days
to be fully processed so, if possible, give us three days notice.

NHS and private prescriptions. Patients on repeat medications can
benefit from their COLLECT DIRECT service. The pharmacies will
collect and process a prescription for you from your local surgery, enabling
the patient to collect it from the pharmacy at a convenient time. The
Pharmacies also offer a free delivery service for patients unable to visit
the surgery/pharmacy. They are also able to offer advice and
information regarding your medication. Should you require any advice
on minor ailments/first aid and injuries/holiday health etc, the Pharmacist
is always available to help. |

to organise blood tests
If the doctor, nurse, or sometimes the hospital, advise blood tests
these can be booked through our receptionists. Our nurses and
health care support workers can all take blood tests. Jenny,
the phlebotomist who has worked with the practice for many years,
is particularly good at getting blood from difficult veins, but no
longer visits the practice: appointments with her can be booked in
the Friday afternoon clinic. This takes place in Rupert Street
Surgery and appointments can be booked by phoning the Path Lab on

 | Results of Tests
Certain results arranged by your doctor can be obtained by ringing reception after 11.30 am.
The doctors view the results of all investigations carried out by the practice. They will comment on them so that you are able to speak to the receptionist for the result. We would ask that you contact us after a suitable length of time so that you can be seen again if needed. We feel that the patient has responsibility for contacting us after investigations.

Obtaining
a sick note from your doctor
If you are off work owing to sickness for up to seven consecutive calendar
days, you need a self certificate (SC2) available from your employer or from
the surgery. If you are self employed, you may collect an SC1
form from the surgery. If you are off work for less than seven
calendar days you do not require a doctor’s certificate. Any
absence for more than seven days, including weekends and bank holidays,
requires an NHS certificate, available from your doctor free of charge.
If your employers require a certificate for a period of less than the
statutory seven days, the doctor is not obliged to issue any certificate but
may issue a private certificate for which there is a fee (current charges
are available from reception).

Making
a complaint
While we constantly strive to provide a good, friendly and efficient
service, we do realise that occasionally things do not go as smoothly
as we would like. Should you have any complaints or suggestions
then please contact the Practice Manager or Senior Partner in person
or by writing if possible.

We are aware of how worrying it can be to have a sick child . If you are concerned about your child we will always see him/her the same day at the surgery. We do ask that whenever possible you bring your child to the surgery rather than requesting a home visit. A child will come to no harm being brought to the surgery and can usually be seen sooner (which sometimes is vital). Your co-operation in this matter is greatly appreciated. It is always wise to keep a supply of children's Paracetamol at home. Paracetamol reduces a child's temperature, so should be given four hourly whenever a child has a temperature. In most minor illnesses in childhood this is the only treatment required. If you are worried about your child, or if your child fails to improve in two to three days it is worth bringing him/her to the surgery for a check

to book hospital appointments using "Choose and Book"
Patients want to be more involved in decisions and making choices about
their healthcare. The NHS is currently trying to give patients a
greater say in how they are treated. This includes giving access to a
wider range of primary care services and increasing choice of where, when
and how to get medicines, treatment and care.
Choose and Book is a
national service that, for the first time, combines electronic booking and a
choice of hospital, date and time for first outpatient appointments.
It will eventually revolutionise the current booking
system with patients being able to choose their initial appointment, and
book it on the telephone or via the internet at a time that is more
convenient for them. As you would expect from the introduction of such
a new system there are teething problems as it is still very much in the
early stages

Volunteer
driver service
A volunteer driver service is available in Biddulph for patients who have
difficulty getting to the surgery, hospital, clinic etc. To
contact them please telephone 01782 518070 between 10.00 and 12.00 week days
to arrange a driver. There is usually a small fee charged for
this facility, and you do need to book the volunteer driver in advance of
your appointment.

Health Advice and Vaccination
We are happy to deal with all enquiries regarding foreign travel, including
vaccinations recommended and general advice. If you are planning to travel
abroad, please contact the surgery 6 to 8 weeks or more prior to travel in
order to allow sufficient time for your required vaccination course. We
provide a wide range of travel vaccinations (except Yellow Fever).

 | Carers'
register.
A carers' file is available in the waiting area to assist patients who are
carers, or patients who are being cared for. The file contains
up to date information and support on a number of issues including
transport, respite care, self help groups, financial and legal affairs,
social services and much more. Our surgery has a carers'
register, if you would like your name to be added to the register please let
a receptionist know.
North Staffs Carers' Association is based in Burslem and is open Monday
10.00am to 6.00pm, and Tuesday - Friday 10.00am to 3.30pm. It is
best to telephone for an appointment before attending the centre.
